RMA Request

In order to return your product to Nextivity for repair you must first submit an RMA request. All returned products are subject to inspection and must be within the warranty coverage period. Please note that physical damage is not covered under warranty.


Instructions: If you would like to return 1 system, fill out the Serial Number and Fault Description below. If you have multiple systems, fill out the Multiple RMA Request spreadsheet and upload it with this form.

Maximum size: 1MB.
Allowed file types: .xls, .xlsx.
